1. List items currently missing from the questionnaire that are needed to achieve the stated survey objectives

a. Gender of the child

b. Identification of the stratum: ID number/Name of the stratum/district/region

c. The number of eligible respondents

d. Start date of enumeration

e. Name of the father and mother

f. They should ask the age of child (if birthdate not known). It seems that form is unclear when to ask this question

g. BCG and IPV vaccines are missing

h. If the Rota has been introduced 1-2 years prior to the survey, then they Rotavirus vaccines should be included in the questionnaire.

i. They should also indicate whether this information is obtained from the vaccination card or from other sources such as caregiver’s recall or health fcailities

j. If the child is present they should check the BCG scar and record the result

k. Instead of asking the name of the vaccine, they should ask location or sites on which the vaccine was administered and whether was given the vaccine in a form of injection or drops and the frequency of administration this way will trigger the memory of the care giver. It is hard to imagine mothers who not literate to recall the name of the vaccine in a foreign language. Some mothers they give vaccines a local name and this can help them remember the type of vaccine received.

Recommend how to improve the alignment between questionnaire and the stated survey objectives

Define the objectives of the survey and list of indicators required to calculate.
Define indicators to determine data elements (enumerators and denominators) required to compute the indicators. In this way the tool will only has required questions and prevent omitting important questions to yield the needed data and information
Proper training of the enumerators on the use of data collection instruments and the data collection technique.
Translating and reverse translating of the questionnaires
List of the local calendars to obtain the age of the eligible children and where possible obtain the local names of the various vaccines
Field test of questionnaires to determine the level of comprehension and easy administration of the tools. This will also help determine the skill gain and competencies of the enumerators.
Selection of well experienced supervisors and providing a quality training on survey methods including data collection techniques and the use of data collection tools.
Including hints in the questionnaire and preparing guidance notes or SOP to guide the data collection process.
