Survey and Research Consent
REQUIRED ACTION: Very Important Announcement!
My name is Adam Rusch and I am a PhD Student in the College of Education. In addition to helping Professor Tapping set up this Immunization Module, I am also looking at the ways that students learn through this online system.
Because we are studying the ways that students learn in this module, we need EVERYONE to read the information at the following link:

Once you have read the information about our research we need you to choose whether you are willing to let us analyze your Case Study and whether you would be willing to take a short survey to give us feedback on the software. If you decline to participate we will discard your Case Study from our research data set when we analyze this class. If you do allow us permission to analyze your work, we will maintain your individual privacy at all times.
I want to make sure it is clear that your participation or decision to not participate in our study will in no way affect your grade, standing in the program, or relationship with the University of Illinois. Also, this information will be kept confidential by the Researchers and not shared with the Instructors of the course.
