  I am a Senior Medical Social Worker at Community Rehabilitation Programme, Tan Tock Seng Hospital, Singapore. I am part of the multi disciplinary team (physiotherapists, occupational therapists, nurses) providing home based therapy services for adults with acquired disabilities (suffering from stroke, spinal cord injuries, and etc) with the aim of reintegrating them back into their community. My role in the programme is to manage the psycho social issues of our clients and help them to attain psycho social wellness. I have been working as a Medical Socail Worker in acute hospital and community healthcare setting for 16 years. I have wide clinical experience working with the elderly population, people with mental health issues, and people with disabilities.
