Abstract
In partnership with Cartrefi Conwy, the focus will be placed on mapping opportunities for innovation by discharge to housing support schemes as an alternative to transfer from acute to community hospital settings. This mixed methods project will seek to find current interventions focusing on transfers from hospital to housing schemes and/or domiciliary care including any challenges or benefits experienced, provide a better insight into the discharge planning process by conducting non-participant observation and collection of available data in key hospital and housing scheme sites across North Wales. It is hoped that this project will add to the discourse of improving the discharge process for patients and family members.
