Investigation of the Nursing Care Needs among Elderly with Cognitive Impairment and Resource Allocation in Shanghai

Abstract

We analyze the nursing care needs, resource distribution, supply and demand matching of elderly with cognitive impairment in institutions in Shanghai, and to provide evidence to policy-making. We selected three districts with moderate economic status to conduct the survey of nursing care needs. Elderly with cognitive impairment and not in acute onset were included in the study. We collected bed distributions of institutions, and adopted a potential model to carry out the calculation of resource supply and demand. The proportion of elderly with cognitive impairment who were willing to receive nursing care need was high, which accounted 80.0% of the study population. However, the gap between resource demand and supply was large, beds supply accounted for 38.8% of the demand and seats supply accounted for 67.5% of the demand. The resource supply distribution was insufficient. Institutions are densely distributed in the urban, while beds and seats are more distributed in the suburbs. The spatial accessibility of beds and seats gradually decreased from urban to suburban. We conclude a need to increase the institution distribution in the suburbs, evenly cover the urban and suburban areas in Shanghai. There is a need to adopt the combination of construction, reorganization and expansion, enhance the utilization efficiency of stock resources, and reinforce the construction of “institution-community-home” cognitive impairment service system.
