1. List items currently missing from the questionnaire that are needed to achieve the stated survey objectives.

To achieve the stated survey objectives following items are missing from the questionnaire –

  • Many necessary questions in sequence are missing e.g.- Q.N – 28, 29, 63, 64, 69, 70, 83, 84, 87 & 104 etc.
  • Identification should clearly mentioned address like country name and provinces and address of child beside longitude and latitude for paper based survey.
  • Demographic variables Missing like –
  • Rural / Urban,
  • Sex
  • Birth order
  • literacy of the mother/caregiver
  • No assessment for BCG & IPV vaccine.
  • Capturing recall based data is missing.

2. List questionnaire items currently unrelated to achieving the stated survey objectives.

Questionnaire unrelated to achieving objectives are-

  • Capturing data for yellow fever vaccine,
  • Rota vaccine question may be removed, as Rota still to implement in Harmonia.
  • Questions regarding charge for issuing duplicate cards
  • Depth of Mobility questions are not directly related to achieve my survey objectives.

3. Recommend how to improve the alignment between questionnaire and the stated survey objectives.

  • Revision of questionnaire keeping objectives in mind will be useful to achieve survey objectives.
  • Chronology of questions should be aligned with RI schedule of Harmonia.
  • No. of questionnaire should be in order (questions no. should not be missing in between).
  • Demographic variables – like sex of child, Birth order, Rural / urban should be added on it.
  • Add BCG & IPV on questioners

4. What steps would you take to ensure that interviewers ask the questions in the same standardized manner?

  • Chronological and systematic framing of questionnaire.
  • Interviewer should be well trained in classroom as well as in field.
  • Piloting of survey to test tools and understanding difficulties of interviewer.
  • Questionnaire should be in simple and understandable language.
  • Supportive supervision and monitoring during survey.
  • Developing specific guidelines for interviewers and supervisors.
  • Develop clear and specific questions with detailed guidance on each question.
  • To resolve any query interviewer should have contact no. of resource person.

5. What factors may contribute to ascertainment bias in this survey?

  • Non-standardized, out dated questionnaire having too many variables.
  • Questions not explained properly on questionnaire.
  • Non rigorous training of surveyor.
  • No field training
  • No Piloting of survey.
  • Lack of supervision
  • Non- availability of guideline for surveyor
  • Questions not specific and interpretable according to the interviewer.
  • Non- availability of cards in majority ( Recall dominance survey)
  • Lack of rechecking data before leaving house and
  • Lack of revisit to incomplete houses
