Abstract
In Colombia, there are successful experiences in Primary Health Care, but the depth of an organized information process that takes up and makes the experience visible from a social perspective is not always accessible. The objective of this project is to systematize the experience of Primary Social Care in the Department of Caldas, with systematization of experience based on FAO guidelines. It articulates the different community actors and sectors, orienting them towards the definition of lessons learned and public policy initiatives. Actors that have participated in the strategy include the community, subjects, and families. Each municipality has given it an integral dynamism that allows it to transcend the health sector and link other areas such as cultural, social, educational, economic, environmental, among others. Initial observations of the implementation of the strategy show that the strategy has been directed by history, regulations, and institutionality. It has been strengthened from the social aspect and the Social Determinants of Health, being led by the municipal administrations. Three axes are intertwined in an articulated way: trans-sectoriality and interdisciplinary education, community social participation, and family welfare and health. Lessons learned from this project include the following topics: the construction of public policy, Social Care Centers, analysis units and a social observatory for the consolidation of information. It is concluded that the orientation of the practice in Primary Social Care has improved the health indicators for Caldas.
