Contemporaneous Model of Service Delivery for Managing Behaviors in Residents of Long-term Care Homes: Results of a Three-year Continuous Quality Improvement Initiative

Abstract

There is a paucity of the results on the evaluation on the Gero-Psychiatry Assessment Teams at-tending to residents with behavioral expressions in long-term care homes. Contemporaneous Model of service, in its present format, has been in existence since 2016 and attending to residents with behavioral expressions in 27 long-term care home in Hamilton, Ontario, Canada. Referral criteria and outcome benchmarks were established. It was able to prevent over 95% of the patients on its active caseload from visiting the emergency department for behavioral risks. The referral of the number of patients on the active caseload of the team to specialized dementia behavioral care unit was extremely low, to begin with, and steadily dropped over the span of three years of follow-up. Geriatric psychiatric assessment team, operating under the Contemporaneous Model of service delivery, is a very effective modality for reducing the burden of the dementia patients with behavioral risks on the emergency departments. Its role in diminishing the burden of Hallway Medicine needs further study.
