Abstract

The major task of diabetes practitioners and clinicians is to support patients in adopting healthy lifestyle changes necessary to mitigate the disease. This will require incorporating well-accepted, evidence-based strategies from numerous research studies into patients’ self-care. The most successful programs are those that demonstrate behavior change rather than merely increasing patients’ knowledge. The purpose of this paper is to identify key behavioral and social science interventions that can assist the diabetes practitioner. We present a conceptual framework for organizing a multi-step approach for providing care that emphasizes principles of behavioral and social science that focus on practical interventions that can be implemented by various clinical practitioners, educators, and policy makers.
